Physical/Parent Consent Form

The Maui Interscholastic League contains three different seasons in the athletics.

Prior to participation in season or off season, all student athletes must have completed a physical and parent consent forms. 

While Physicals are good for 1 year from date taken, Parent Consent forms are needed for each sport participating in during the year and are needed to be turned in to school's Athletic Trainer and/or Head Coach.
